2. 投标人如为所投产品的制造商，须提供制造商声明函；投标人如为所投产品的代理商或经销商，须提供制造商出具的针对本项目的合法授权文件或合法的代理证明文件加盖公章；

3. 若投标人为所投产品的制造商，国产制造商必须提供《医疗器械生产企业许可证》，且生产范围包含该产品；若投标人为所投产品的代理商或授权供应商，必须提供《医疗器械经营企业许可证》，且经营范围包含该产品；（境外投标人请提供公司注册地规定的相关资质（如有）。）；

4. 投标人必须提供所投产品的《医疗器械注册（备案）证》的复印件，原件备查（开标时，该证应在有效期内；若不在有效期内，则需提供该证和所投产品在该证有效期内生产的药监局出具的证明文件）；

5. 投标人必须提供由设备制造商或授权的中国总代理签署的合法有效的保修、维修承诺函；

6. 证明投标人已具备履行合同所需的财务、技术和生产能力的文件；

7. 投标人基本账户开户银行在开标日前三个月内开具的资信证明原件或该原件的复印件；

8. 投标人须提供设备在国内的销售业绩表或合同复印件或中标通知书复印件（表中应列出设备的出厂日期、用户名称、地址、联系人等资料）；

9. 投标人应当于招标文件载明的投标截止时间前在中国国际招标网（以下简称“招标网”，网址：http://www.chinabidding.com）成功注册并完成信息核验。否则，投标人将不能进入招标程序，由此产生的后果由其自行承担。
